Quigley update. Her surgery site failed miserably and is infected. Mouth surgeries are hard to keep clean. She is staying till Thursday at least. She is on IV antibiotics, pain meds, and sedation meds. They are keeping her quiet and calm. Her surgeon and I spent forever on the phone today and yesterday. I gave her every suggestion you all gave to me and we worked through them and most are not adaptable to her specifically and or just fundamentally won’t work on her. Some summaries of suggestions:
Dr. Rausch has consulted with pretty much everyone already. I know she’s consulted some other colleagues out at other places but also the 20+ specialists they have at VSS have all consulted with her about Quigley in house a few times. For the Love of Ruby’s mom and I talked yesterday. Her Philadelphia cleft surgeon that did Ruby’s surgery is using the same technique as Dr Rausch is with the flap and besides Ruby’s healed with no issues and it only took the one surgery. Same with mizzou and most other vet schools, same flap method. Dr R even found this cool thing about retainers in a new study/journal but Quigley does not have teeth, she had to have all the uppers removed because there wasn’t enough skin for a flap to begin with. So there’s nothing to affix the retainer too and her bone structure is not conducive for putting in posts of any kind and they are highly prone to get rejected and infected. And on top of that we have a major infection going currently. And on top of that every time she’s under anesthesia she has major intestinal problems and stomach problems and regurgitates everything for quite a long time after once she finally starts eating. And on top of that we have a patient that’s not amicable for anybody messing with her face anymore.
Probably biggest part of the biggest problem is the fact that the entire front nose portion of her face is not affixed to anything and can’t be. They did this specialized 3d imaging and there is no way to “mount” her nose. We can’t put her in a sedative coma like they can with humans. We can’t wire her jaw shut, she keeps using her tongue to lick the roof of her mouth. She could still lick even with it wired shut. We can’t muzzle her cause that allows way too much movement, way more than wiring it shut. Ugh. We can’t put in a feeding tube because she is sick of being messed with, she eats too much for it to be feasible with pushing the liquids, she freaks enough to make herself vomit. She flat refuses anything and forcing her makes her vomit. We can’t use her tonsils or pigs intestines as a donor skin graft because the site needing repair is too far away for the donor skin to stay healthy and it usually breaks down because the body rejects it as a foreign body. Her soft palate healed nicely, the problem is the hard palate. We can’t use cadaver skin of any type because this particular area is hard to keep oxygenated. We can’t use glue or mesh cause the body won’t allow the “bandaid” to stay in place long enough to heal. She has no more available viable skin to use unless her lip is used which is terribly iffy and painful.
If anyone has a HUMAN cleft surgeon in the family please have them contact us and maybe there is something they are doing with humans that could work here? We feel this is an extremely big shot in the dark because most human cleft surgeries are actually more so cosmetic.
We had a hard discussion about what we do if we can’t fix the cleft. Quigs can not continue like this. If you have seen her gag, choke, throw up, panic and run to you because she can’t breathe it will break your heart. I refuse to process this fully. I am already getting close to Zayda’s quality of life recheck with her calcium buildup degenerative issues and inability for mobility. I can not loose both. We just can’t. It isn’t FAIR. We need a miracle. Tomorrow is Quigley’s recheck to see if any of the newly failed flap is salvageable or if it is too infected. Paws crossed it is better and some of it can be saved and repaired tomorrow. Please send her some good if you got some to spare.
