Hello, my name is Papi Parcheesi.

I am a neutered male white Maltese at the Manhattan Animal Care Center.
The shelter thinks I am about 9 years old.
I only weigh 18 lbs.
I came into the shelter as a owner surrender on 10-Jan-2019, with the surrender reason stated as personal health - medical.
Behavior Condition: 1. Green

Papi is described as friendly, affectionate, mellow and excitable. He enjoys playing and laying with his favorite stuffed animal, was being fed Pedigree wet and dry food mixed 3 times a day and was being walked 2 times a day.
Papi is a roughly 9 year old small dog whom was adopted from ACC and kept in the same home for roughly 1 year.
Unfortunately due to the owner's health he was unable to be kept.

History: O reported that since Saturday he has not been able to walk.
Everything was normal up until this time.
His coat is light blue owner reports because of new sheets that the stained was rubbing off.
Subjective:BAR, 2-3% dehydrated
Observed Behavior -Friendly. Allows all handling.
Evidence of Cruelty seen -No
Evidence of Trauma seen -No
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: 3-4/4
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, no c/s
ABD: Non painful, difficult to palpate deeply due to body condition score, no masses palpated
MSI: Ambulatory x 2, he has motor in hindlimbs but minimal, prefers to lay down, no obvious pain/fractures/instability/abnormal joint effusion noted in hind limbs, skin free of parasites, no masses noted, healthy hair coat, coat light blue from new sheets
CNS: Alert and appropriate, decreased menace OD but otherwise normal cranial nerves, paresis both hindlimbs but does have mild motor, CP deficit RHL, normal pain response, normal anal tone, normal cutaneous trunci, normal wheelbarrowing, decreased placement, NO obvious pain neck or back
Rectal: Clean externally
Assessment: Weakly ambulatory hindlimbs (right>>left) with mild motor and normal anal tone (NOT obviously painful)-r/o vascular event/FCE (from hypoT4 vs. Cushings vs. other hormonal vs. other) vs. other neuro (IVDD vs. infectious vs. inflammatory)
Overconditioned-r/o hypoT4 vs. Cushings vs. overfed
Send TSH to idexx to confirm thyroid status
Buprenorphine 0.6 mL IM (just in case he is painful and not showing signs)
IVC LRS @ 32 mL/hr (90 mL/kg/day)
Start gabapentin 200 mg/mL-Give 0.4 mL PO BID x 7 days.
Give him 48-72 hours (unless he declines) to see if he improves and if not recommend EHR.
If he does respond likely vascular event/stroke/FCE and will need to determine underlying condition and treat it so not a recurring issue. Addendum: Decreased IVF rate to 21 mL/hr (60 mL/kg/day) @ 3pm because he feels hydrated.
Total T4: Low normal @ 1.1 (reference range 1-4).
TSH pending at idexx.
