INTAKE NOTES, DATE OF INTAKE 8/9/2018: Upon intake Chopper was super friendly and sweet allowing me to pet him. Chopper allowed all handling. While in the lobby it is reported that Chopper was reactive to another animal in the lobby while owners restrained him.
OWNER SURRENDER NOTES – BASIC INFORMATION: Chopper is a neutered male large mixed breed. Owners got Chopper from a friend and has had him since he was a puppy. Owners are surrendering due to Choppers reactivity towards animals has worsened. Chopper has no health problems. Chopper previously lived with children and adults. Around strangers Chopper is friendly/ outgoing. Chopper has spent time around children ages 1-16. Around children Chopper is relaxed and playful. Around other dogs Chopper is aggressive. He is reactive to other animals. Chopper has never been around cats. He is reactive to other animals. When someone touches Choppers food or bowl while eating he isn't bothered. When someone takes aware a bone, rawhide, or treat away Chopper isn't bothered. When pushed or pulled off furniture chopper isn't bothered. When being held or restrained he isn't bothered. If disturbed while sleeping or resting he is not bothered. When given a bath or being brushed Chopper isn't bothered. When someone trims Choppers nails or touches his feet he is afraid. When someone unfamiliar approaches the house or yard Chopper barks. When someone unfamiliar approaches owner or family member chopper isn't bothered. Chopper bit a 16 year old 2 years ago while at the park with family. Chopper became over stimulated and bit the 16 year old on the hand left a mark, did not break skin. Chopper is housetrained and has a medium activity level.
For a New Family to Know Chopper is described as a friendly, playful, excitable canine. Some favorite things about chopper is that he is a sweet dog with people and is a big mush. Chopper favorite activites playing with a ball or getting scratched. When the owner his home Chopper tends to follow you around. Chopper enjoys balls, chew/bones. Chopper likes to play tug and wrestling. This dog has been kept mostly indoors. Chopper likes to sleep everywhere. This dog eats both wet and dry food. This dogs favorite treats are Chicken. Chopper is partially house-trained. Chopper has accidents at home daily. Chopper usually goes potty on or in grass. When left alone this he goes potty indoors. Chopper is never left in the yard. Chopper has never been crate trained. Chopper knows sit, come, down. For exercise this dog is used to slow walks on the leash or plays in the yard. On the leash this dog pulls very hard. When off leash this dog runs away.
PLAYGROUP NOTES – DOG TO DOG SUMMARIES: Chopper is reportedly highly reactive to other animals. Owner stated Chopper has become difficult to walk due to his reactivity.
8/10: When off leash at the Care Centers, Chopper greets the novel female with a tense, square posture, hackles raised. He tolerates her sniffs but continually follows her around, focusing on her genitals. He does not respond to her solicitations and briefly explores the yard.

Sensitivity:: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog's mouth is likely closed for at least a portion of the assessment item.

No cats: Owner reported Chopper has displayed some reactivity toward other animals, and poses a potential safety risk for cats. We advise Chopper be placed in a home without resident cats, or where he may come into contact with cats.
Single-pet home/Recommend no dog parks: Due to the concerning behaviors that Chopper has shown during at home, we feel that Chopper should not visit dog parks and be the only resident dog. The Behavior Department recommends that he be socialized in a more controlled setting until his behavior towards other dogs can be further addressed. Reward-based, force-free training can be utilized to help Chopper associate dogs with things he enjoys like toys or treats.

History: Intake 8/13-geriatric, overweight, otitis externa, dental disease, skin masses. Gave claro at intake.
8/18-started on baytril and doxcycline for CIRDC.
Subjective: QAR. Noted on rounds board to have vomited multiple times yesterday. He is very lethargic and not eating well. He threw up his oral meds yesterday. No csvd but is not moving around the cage.
Objective:
EENT: serous nasal d/c bilaterally, no ocular d/c ou
L: Eupneic, normal RR/RE
MSI: Ambulatory x 4, good haircoat
