The officer kept me for 19minutes causing me to loss my new job. Now I would like to know can he help me pay rent or a car payment, or will he even care if I'm homeless at the end of the month since I have been looking for work for 3months! Your claims are very serious but more alarmingly very false. You were stopped for traveling 57MPH in a 45MPH Zone in which you received a WARNING. You failed to keep insurance on your vehicle which is cause for your vehicle to be towed. the very generous police officer allowed you time enough to contact your insurance agency and update your insurance policy by phone, to which you did and were then allowed to leave. So, to sum up, this officer gave you a warning for speeding, didn't tow your car and allowed you to contact your insurance to update your policy. Was there anything else this officer could have done for you? Perhaps you meant to score our review higher? I’m apparently to blame when Kathy can’t figure her sleep schedule out.: Using the 5 Whys tool, review each cause until the team reaches the root cause for each of the problems being addressed. List the root causes. Why did Kathy turn off the alarm instead of snooze? Because she hit the wrong button on the alarm. Why did she hit the wrong button on the alarm? Because she was too tired to open her eyes to look so she guessed. Why was she so too tired to open her eyes? Because she was working late on a project that was due in the morning. Why was the report late? Kathy has to put in an IT request to get the report. Why does Kathy have to put in an IT request? Because Janet is on maternity leave and doesn't have backup. Root Cause: Janet doesn't have backup for running scheduled reports when she is out of the office. Why did Kathy forget to set the alarm? Because she was too tired. Every day she'd be late to work because she was convinced that her hair dryer was going to burn down the house so would always have to turn around and check it. Multiple times a day even. A bunch of doctors tied to "fix" her of that fear, until one day she got a doctor that suggested she bring the hair dryer with her. Other doctors were annoyed, saying that wasn't a the correct way to help, but she gave it a go. When she had that fear, she'd look over and see the hair dryer unplugged in the seat next to her and was able to carry on.
